Once completed, please print and have the form signed by:

  • yourself
  • your parent/caregiver
  • the teacher of the subject
  • Academic Leader (Years 10-12 only)
  • Deputy Principal - Learning & Teaching (Years 11-12 only)

"Guidance Notes" and "How to Submit this Form" information appears on page two of the form.

Special consideration must be applied for in advance, NOT on the due date.
