Subject change forms can be downloaded and completed electronically.

Once completed, please print and have the form signed by:

  • yourself
  • your parent/caregiver
  • the teacher of the subject you are leaving
  • the teacher of the subject you are entering
  • the VET Coordinator, if applicable

and lodge at Student Reception.

NOTE:  Students must receive their new timetable before changing to their new subject.
