Once completed, please print and have the form signed by:
- yourself
- your parent/caregiver
- the teacher of the subject
- Academic Leader (Years 10-12 only)
- Deputy Principal - Learning & Teaching (Years 11-12 only)
"Guidance Notes" and "How to Submit this Form" information appears on page two of the form.
Special consideration must be applied for in advance, NOT on the due date.
